There are many various records of your academic career required by the Department, the Graduate School and the University. To help you navigate this, we've gathered the documents containing important policies and procedures that students should know on this page. The documents linked below will be used and referenced frequently by all students.  Please take the time to familiarize yourself with the different sections of this page. You’ll find that many of your questions can be answered by the documents linked below.

Graduate Documentation:

  • Curriculum Development Plan (CDP) Outline [Hardcopy| DocuSign PowerForm]
    • The intent of this document is to outline the courses the student may take to complete their graduate degree. This document identifies courses that the student holds interest in, the professor would like the student to take and the required coursework. This plan is a roadmap drafting the plan to completion with required credit hours.
  • Curriculum Development Plan (CDP) Record of Completion. [Hardcopy| DocuSign PowerForm]
    • This form documents the courses the student did complete to meet the requirements of their degree.

All committee signatures should be submitted at one time.

A multi-page document is acceptable; however, all signatures should be submitted at one time. The student or supervising professor is responsible for obtaining these signatures.


DocuSign is available to ALL University of Utah constituents. Information about this is available here. The Department has generated templates for all the forms listed above. These templates are the "DocuSign PowerForms". These documents are prepared to be completed by the student.

** Important Information

There are limitations to the systems available for these forms.

  • The preferred method is to use the templates created within DocuSign. These templates are set-up to be started by the student.
  • Hardcopy- requires that all are present in person.
  • Fillable PDF. Not set-up to gather signatures.

The Graduate Advisor is always working to simplify this process for all those involved. Currently the University does not have adequate systems to complete these documents with ease. University IT and the Graduate School have a large project underway to increase the ease and efficiencies of graduate tracking.
